症例は69歳，男性．近医でパーキンソン病，肺気腫およびアルコール性肝障害などと診断 され加療中であった．突然，心窩部痛を認め，改善しないため当院に救急搬送となった．心窩部に圧痛を認めたが腹膜刺激兆候はなく，腹部造影 CTで総肝動脈解離と診断し同日入院となった．
心窩部痛は持続していたが腸管虚血所見を認めなかったことから，アセトアミノフェン点滴による疼痛コントロール，絶食による腸管安静や補液を行い翌日には症状が改善した．その後も症状の再燃なく，第８病日の腹部造影 CTでは総肝動脈解離の偽腔における血栓像は縮小し，末梢血管の血流は保たれていたため同日に退院となった．孤立性腹部内臓動脈解離は稀な疾患であり，特に孤立性総肝動脈解離の報告例は少ない．孤立性総肝動脈解離を含む腹部内臓動脈解離は中高年の男性に多く，強い腹痛に対し腹膜刺激兆候を認めないことが特徴である．本症例のように腹部所見が乏 しく突然発症の腹痛を認める患者では，孤立性総肝動脈解離を含む腹部内臓動脈解離も急性腹症の 鑑別疾患の一つとして考慮する必要がある． |

The 69-year old male patient was treated at a nearby clinic for Parkinson's disease, pulmonary emphysema and alcoholic liver injury. Due to sudden onset and persisting epigastric pain the patient was admitted by an ambulance. Physical examination did not reveal signs of peritoneal irritation but only epigastric tenderness. Contrast-enhanced computed tomography of the abdomen revealed common hepatic artery dissection. This case was successfully managed by conservative medical therapy with pain control treatment and fasting. Symptoms were alleviated by the next day, and he was discharged on day 8. He did not develop common hepatic artery dissection or organ ischemia during follow-up. Contrast-enhanced computed tomography of the abdomen on day 8 showed a regressed false lumen and a dilated true lumen at common hepatic artery dissection. Peripheral blood flow was maintained without organ ischemia. Spontaneous isolated dissection of a visceral artery is rare and there are very few reports about common hepatic artery dissection. It is characterized by the absence of signs of peritoneal irritation, although severe subjective symptoms are present. Patients with spontaneous isolated visceral artery dissection are predominantly middle-aged and elderly men. In patients with sudden onset of abdominal symptoms with minor abdominal findings, we suggest considering spontaneous isolated visceral artery dissection including common hepatic artery dissection as a differential diagnosis. |
