(201TI SPECT) in combination wigh CT, MRI, or 18F-FDG PET/CT has been used to evaluate intracranial tumors, but previous reports on the clinical value of 201TI SPECT for their differential diagnosis are limited. In this study, to re-evaluate the usefulness of 201TI SPECT, we retrospectively analyzed 39 patients (25 males, 14 females; age, 64 ± 13 years) with an intracranial tumor who underwent 201TI SPECT to differentiate benign and malignant tumors at our institution from April 2014 to March 2019, showed visually significant 201TI accumulation in the tumor, and whose clinical diagnosis was finally determined. \n　Of the 39 patients, 26 patients were finally diagnosed with an intracranial malignant tumor, and other 13 patients with an intracranial benign tumor, by pathological examination, clinical follow-up (> one year for benign tumors), or therapeutic diagnosis. From 201TI SPECT images, the accumulation ratio of the tumor (T) to the normal brain (N) in the early phase (early T/ N) and the retention index (R.I.) from early T/N to late T/N were calculated and compared between benign and malignant tumors, and between their histological types. \n　We found that the early T/N tended to be moderate (5.77 ± 4.60) in malignant tumors, and low (2.44 ± 0.91) in benign lesions, except for in meningiomas, which showed very high values (11.91 ± 9.45). The R.I. tended to be high (1.03 ± 0.54) in malignant tumors and low (0.63 ± 0.31) in meningiomas, but relatively high (0.93 ± 0.25) in benign lesions, except for meningiomas. When the cut-off value of R.I. for malignant tumors was set at > 0.7, the sensitivity was 69.2%, specificity was 38.5%, and accuracy was 59.0%. \n　In conclusion, early T/N and R.I showed different tendencies depending on the tumor histological type, and the combination of these variables could be useful for the differential diagnosis. However, it should be noted that these values could be influenced by various factors, such as tumor size and internal SPECT）は頭蓋内腫瘤の評価目的で，CT や MRI，18F-FDG PET/CT とともに現在も用いられるが，その鑑別診断における有用性の報告は限られている．今回我々は，2014年４月1日から2019年３月31日の間に，自施設で頭蓋内腫瘤の鑑別診断を目的として201TI SPECT が施行され，視覚的に有意な集積を認め，かつ臨床的に最終診断に至った39例（男性25例，女性14例，平均64歳）を対象として後方視的に解析し，その有用性を再評価した． 　\n　病理検査，臨床経過（良性の場合は１年以上）または治療的診断によって，39例のうち26例は 悪性腫瘍，13例は良性病変と最終診断されていた．201Tl SPECT 画像からは，早期相における腫瘍 （Tumor）と正常脳（Normal）との集積比（早期 T/N 比），および後期 T/N 比と早期 T/N 比の比 である Retention Index（R.I.）を算出し，腫瘤の良悪性や組織型によって比較した．\n　早期 T/N 比は，髄膜腫で11.91 ± 9.45と非常に高く，悪性病変では5.77 ± 4.60と中程度で，髄 膜腫以外の良性病変では2.44 ± 0.91と低い傾向を認めた．R.I. は，悪性病変では1.03 ± 0.54と高 く，髄膜腫では0.63 ± 0.31と低い傾向を認めたが，髄膜腫以外の良性病変では0.93 ± 0.25と比較 的高値を示した．R.I. のカットオフ値を0.7以上とした場合の悪性腫瘍の感度は69.2％，特異度は 38.5％，正診率は59.0％であった． 　\n　早期 T/N 比や R.I.は腫瘤の良悪性や組織型によって異なる傾向が見られており，これらを組み 合わせることで診断の一助になり得た．ただし，早期 T/N 比や R.I.
