基底細胞癌は，露出部位，特に顔面・頭頚部に好発する腫瘍で，臀部に発生することは稀である．今回我々は，肛門周囲皮膚に発生した基底細胞癌の１例を経験したので若干の文献的考察を加え報告する．症例は63歳，女性．肛囲４時の方向に20×35mm 大の腫瘤を認め，皮膚科で生検を施行され基底細胞癌の診断であった．手術目的で当科紹介となった．当科にて単純切除し術後経過良好であった．肛門周囲で発生した基底細胞癌の場合，肛門管類基底細胞癌と鑑別する必要がある．肛門管類基底細胞癌は予後不良であり切除範囲も大きく異なるため，念頭に置きながら鑑別をしていかなければならない |

Basal cell carcinoma is a tumor commonly found on exposed areas, such as the face, head, and neck, and rarely found on the buttocks. In this report, we describe a case of perianal basal cell carcinoma, with some literature discussion. The patient is a 63-year- old woman. A 20×35 mm mass was found in the 4 o’clock perineum angle, and biopsy was performed at our hospital’s department of dermatology. The patient was referred to our department of surgery. The patient underwent a simple resection at our department and was well postoperatively. Perianal region basal cell carcinoma should be differentiated from anal canal basal cell carcinoma. Basaloid carcinoma in the anal canal has poor prognosis, and the resection extent is extremely different; therefore, the distinction must be made with this noted. |
