介護福祉士が喀痰吸引や経管栄養等の医療的処置（「医療的ケア」と呼ぶ）を行うことが一定の 条件のもとで可能となったことを受け，平成24（2012）年，介護福祉士養成校のカリキュラムに， 履修要件として「医療的ケア」科目が導入されることになった。介護福祉士養成校は，「医療的ケア」 を「日常生活を継続するための支援行為」であり介護福祉士の専門性を特徴づけるものとして導入 を進め，教育手法等について試行錯誤を繰り返している。これまで養成校の課題となっているのは， 介護福祉士の専門性を生かして「医療的ケア」を行うことの意義・意識づけを強化する教育と，養 成課程における実地研修の実施である。特に，実地研修は卒業後就労するなかで受講することも認 められていることから，多くの養成校において未実施状態であることが課題となっている。
川崎医療短期大学医療介護福祉科では，平成26（2014）年から「医療的ケア」の講義を開始し， 令和３（2021）年には従来の２年制教育から３年制教育へと移行して，養成課程のなかで実地研修 を実施することとなった。そのなかでカリキュラムを再編成し，介護福祉士の専門性への意識づけ の強化，基本研修から実地研修への「橋渡し教育」の導入，実地研修先との連携に重点的に取り組 んでいる。「橋渡し」科目においては，介護福祉士による医療的ケアに対する理解を深め，コミュ ニケーション能力を育成し，介護技術の修得を確認することを目指す。実地研修先との連携では， 実習の効果を高めるため実習指導者連絡会や本学教員による実習巡回において現場の指導看護師と の連携を深める。さらに，これらの取組を検証し教育の質の保証に努めていくため，「医療的ケア」 に特化した PDCA サイクル体制を構築した。 |

In 2012, the revision of the Law on Social Workers and Certified Care Workers made it possible for certified care workers to perform medical procedures such as sputum aspiration and tube feeding (referred to as‘medical care’) under certain conditions. Since then, training schools for certified care workers have been working on the development of educational methods so that students can perform safe and effective‘medical care.’Particularly, they have encouraged students to be aware that‘medical care’provided by care workers is different from that by nurses. They regard‘medical care’by care workers as a support action to help patients continue their daily lives, which characterizes the specialty of care work. They also have been making efforts to introduce practical training at facilities or hospitals into their curriculum. So far, however, very few schools have offered such practical training.
The Medical Care Work Department, Kawasaki College of Allied Health Professions started lectures on‘medical care’in 2014, and in 2021 it changed the curriculum from two-year to three-year in order to carry out practical training at hospitals. We focused on education to raise awareness of the specialty of care work,‘bridge education’from basic training to hospital training, and cooperation with training facilities. In the‘bridge’subjects, students can understand the significance of‘medical care’by care workers, and acquire communication skills and reliable care skills. A liaison meeting with nurses in charge of training will be held once a year, and information will be shared between nurses and teachers during the training period so that the effectiveness of training can be enhanced.
In order to verify these efforts and strive to further ensure the quality of education, we have established a PDCA cycle system specializing in‘medical care’education. |
