Borrmann II型進行癌と鑑別困難な早期胃癌の5例を経験した.そのうち,3例は男性で,2例は女性であった.年齢は,49歳から57歳までに及び,病変は,胃前庭部に存在した.大きさは2.1×1.8cmから4.1×3.7cmであった.上部消化管造影では,中心部に陥凹を有し,その周辺部は隆起していた.内視鏡検査では,不整な潰瘍を認め,周辺部は盛り上がっていた.不整な潰瘍より胃癌との診断は容易で,生検にて確認された.しかし,その深達度診断は困難であった.深達度診断には,胃壁の硬化像,大きさ,隆起部の高さ,陥凹部の深さが重要な因子である.自験例では,陥凹部は浅いが隆起部は高く,病変は大きく,そのため病変が一塊となり深達度診断を誤り,Borrmann II型進行胃癌と誤診された.組織学的検査では,深達度smの早期癌であった.さらに深達度診断に対しての努力が必要である. |

Five cases of early gastric cancer, which were difficult to differentiate from Borrmann type 2 advanced gastric cancer, were experienced. Three were male and two were female. They ranged in age from 49 to 57 years. The lesions were located on the antrum of the stomach ranged in size from 2.1×1.8cm to 4.1×3.7cm. Roentgenograms of the upper gastrointestinal series showed them to have a central depression and elevated circular margins. Endoscopical examinations showed ulcers, which were irregular, and the surrounding area was elevated. It was easy to diagnose the lesions as gastric cancer because the ulcers were irregular and histological examinations of biopsied specimens obtained endoscopically confirmed the diagnosis. But it was difficult to detect the depth of invasion of the cancer. For the detection of the depth of invasion, gastric wall rigidity, size, the height of the elevated margins and the depth of depression were important factors. In our cases, the depth of depression was shallow but the height of the elevated margins was high and the size was large. Because mass formation of the lesions gave the lesions stiffness, the depth of invasion of the lesions was misdiagnosed, and the lesions were misdiagnosed as Borrmann type 2 advanced gastric cancer. Histological examinations of the resected specimens showed the lesions to be early gastric cancer with a depth of invasion of sm. We must make more effort to detect the depth of invasion correctly. |
