近年，日本の医療施設が外国人患者を受け入れる機会が増加し，外国人患者が医療機関受診において経験するコミュニケーション・ギャップに関する報告が散見されている。そこで，本研究は，外国人患者が経験するコミュニケーション・ギャップについて調査し，医師が養うべきコミュニケーション能力を明らかにすることを目的とする。具体的には，AMDA国際医療情報センターで外国人患者からの相談に対応している相談員45名を対象に質問紙調査を行った。さらに，質問紙回答者のうち，無作為抽出された相談員3名に対し，面接調査を行った。その結果，コミュニケーション・ギャップは，「日本語能力の不足」，「外国人に対する偏見への恐れ」，「医師の説明不足」，「文化・慣習の違い」であることが示された。医師が習得すべきコミュニケーション能力として，「簡潔，論理的な説明」，「文化・慣習の違いの認識」，「思いやり，共感力」などが挙げられた。これらのことから，「簡潔，論理的な説明」のためには，日本語，英語を母語としない外国人にとって，平易な表現とはどのようなものかについて言語学的に理解する必要があることが明らかになった。これによりコミュニケーションが可能になり，「文化・慣習の違いの認識」が深まり，さらに，「思いやり，共感力」を持つことが可能になると考える。 |

Background: In recent years, there have been increased opportunities for Japanese medical institutions to have foreign patients, and communication issues experienced by foreign patients have been reported.
Objective: The present study aims to examine the communication issues faced by foreign patients, and to identify communication skills that medical doctors should acquire.
Methods: A questionnaire survey was administered to 45 research participants who were engaged in telephone consultation for foreign patients at the Association of Medical Doctors of Asia (AMDA) International Medical Information Center, followed by interviews with three randomly selected persons among the above mentioned research participants.
Results: The major communication issues experienced by foreign patients included the lack of Japanese language proficiency, fear of prejudice against foreigners, insufficient explanation by medical doctors, and differences in customs and cultures. Concerning the communication skills necessary for medical doctors, concise and logical explanation, adequate awareness of cultural differences, and capacity to empathize were mainly highlighted.
Conclusions: It is necessary for medical doctors to have linguistic understanding of appropriate vocabulary that can be comprehended by foreign patients whose native language is not Japanese or English so as to provide concise and logical explanation. This would enable medical doctors to communicate with foreign patients smoothly, to deepen their awareness of cultural differences, and to develop capacity to empathize. |
